Comment

Various aspects of this study deserve comment, the first one being the remarkable effect on portal pressure attained with losartan. The mean reduction in HVPG of approximately 45% is the most pronounced pharmacological effect on portal hypertension described so far in cirrhotic patients. Before this study, the greatest reduction in HVPG had been 36%, which was reported in an early study of long-term oral administration of propranolol in cirrhotic patients (Hepatology 1982;2:523-527).
