Special aspects of right colon carcinoma

Summary

752 patients with colon carcinoma of the years 1966–1985 were studied retrospectively. 189 tumors were located in the right and 563 in the left colon. There was no difference in the 5-year survival rate, Dukes classification and operative mortality in both groups. Considering the higher median of tumor diameter, the three months longer diagnostic delay and the twice higher rate of undifferentiated carcinomas the prognosis of the right sided tumors is relatively better. In our opinion the cause of this more efficient local tumor defence is the better blood and lymph supply of the right colon and mucosal mechanisms. Expression of this better tumor defence is the distinct tumor colitis in the right colon. Improvement of the right colon carcinoma prognosis is to be expected of earlier tumor diagnosis. Routine endoscopic evaluation of hemocult positive subjects is most helpful.

Zusammenfassung

In einer retrospektiven Studie von 752 Patienten mit Coloncarcinomen der Jahre 1966 bis 1985 konnten 189 Tumoren des Rechtscolons und 563 Linkscarcinome erfaßt werden. Die globalen 5-Jahres-Überlebensquoten, das Stadienraster nach Dukes und die Operationsletalität waren in beiden Gruppen gleich. Bei einer höheren durchschnittlichen Tumorgröße, um 3 Monate längeren diagnostischen Verschleppungszeit und doppelt so hohen Rate an entdifferenzierten Carcinomen muß der Verlauf des Rechtstumors dennoch als relativ gutartiger eingestuft werden. Als Ursache für diese relativ bessere, lokale Tumorabwehr werden die dichtere Vascularisierung und Versorgung des Rechtscolons mit Lymphgefäßen sowie mucosale Mechanismen diskutiert. Ausdruck dieser angenommenen Abwehrreaktion ist eine nur im Rechtscolon beobachtete, heftige Tumorcolitis. Im Bestreben um die Verbesserung der Behandlungsergebnisse der Rechtscolontumoren kommt der Carcinomfrüherkennung, vor allem durch breite Ausschöpfung endoskopischer Möglichkeiten bei hämoccultpositiven Patienten, besondere Bedeutung zu.
