Caroli's disease: A premalignant condition?

Abstract

This study reports four new cases of Caroli's disease complicated by the development of cholangiocarcinoma. Caroli's disease appears to be a premalignant condition, but early diagnosis of malignancy in the cyst wall is difficult because signs of ductal stenosis develop slowly and treatment is often based on erroneous diagnosis of recurrent cholangitis. When carcinoma is present, operative intervention makes no difference in survival. In patients with Caroli's disease, adequate intraductal and hepatic biopsies should be obtained at the first operation. These patients should be followed carefully, and malignant growth should be suspected when cholangitis occurs despite patent anastomoses. Earlier diagnosis will obviate repeated operations in patients with malignant Caroli's disease.

      This is considerably higher than in the general population (prevalence rates 0.05%; incidence rates of 0.5-1.5 per 100,000 person-years).216,219 The reason is unclear but may be related to the carcinogenic effects of chronic inflammation due to recurrent cholangitis,220 carcinogenic effects of biliary stasis,216 or formation of carcinogens due to permanent biliary irritation caused by gallstones.221 Pre-operative detection of cholangiocarcinoma is difficult in patients with Caroli disease and syndrome because of the abnormal anatomy associated with biliary dilatation.216,222,223

      Unfortunately, due to the small patient sample without prior endoscopic procedures in this study, the comparison between patients with and without endoscopy could not undergo valid statistical analysis. The risk of developing intrahepatic cholangiocarcinoma is increased by 100-fold in patients with CD/CS [29]. Furthermore, it is difficult to detect a tumor within the dilated biliary tract, making early detection almost impossible.
