Abstract

This is a case report of a 7 year old patient with acute noncalculous cholecystitis due to gastric heterotopia in the gall bladder.

Heterotopic gastric mucosa in a duplicate gallbladder

A 7-year-old girl presented with recurrent acute pancreatitis. Detailed investigations, including endoscopic retrograde cholangiography, suggested a poorly defined biliary tract abnormality. At laparotomy, this was discovered to be a duplicate gallbladder each with a separate cystic duct but contained within a single serosal envelope. Both gallbladders were removed, and histologic examination found the inferior organ to be lined by heterotopic fundic-type gastric mucosa. Despite the absence of any remaining structural biliary abnormality and no evidence of residual ectopic gastric mucosa, the patient experienced a few further episodes of self-limiting mild acute pancreatitis during the following 3 years. The presence of heterotopic gastric mucosa in a duplicate gallbladder has not been described previously.

We report a case ofgastric beterotopia in the gallbladder of an 18-year-old man. Symptoms of acute cbolecystitis are characteristic in patients under 20 years old, while older patients present with chronic cbolecystitis and gallstones. The beterotopic mucosa results either in a mass, a polyp, or a multiloculated gallbladder. Twenty-eight other cases of gastric beterotopia in the gallbladder or cystic duct have been reported. In only three was it associated with peptic ulceration. Treatment is cbolecystectomy.
Heterotopic gastric mucosa has been described throughout the length of the gastrointestinal tract from the oral cavity¹¹ to the rectum¹⁰. Curiously, it is extremely rare in the gallbladder, where it has the propensity for causing symptoms of acute cbolecystitis, particularly in the young. We report a case of this condition in which there was a separate loculus lined by gastric epithelium.
Heterotopic gastric mucosa in the gallbladder: A cause of chronic abdominal pain in a child

A 12-year-old boy presented with a 7-year history of intermittent colicky epigastric pain. Serial abdominal ultra-sound studies showed a contracted gallbladder, and single and double dose oral cholecystograms demonstrated nonfunction. All other investigation results were normal. At cholecystectomy, there was a fibrotic stricture at the junction of the gallbladder neck and cystic duct. The small, thin-walled gallbladder contained white bile. Histologically, the gallbladder was lined with normal mucous secreting columnar epithelium. The area of stricture showed diffuse fibrosis with islands of heterotopic gastric mucosa containing all gastric mucosal cell types. One year postoperatively, there have been no further episodes of abdominal pain. This is the seventh reported case of heterotopic gastric mucosa in the gallbladder of a child causing symptoms necessitating cholecystectomy. We recommend that a child with abdominal pain and a nonfunctioning gallbladder be considered for cholecystectomy even in the absence of cholelithiasis.
