This literature review and the recommendations therein were prepared for the American Gastroenterological Association Clinical Practice and Practice Economics Committee. The paper was approved by the committee on September 27, 1998.

History

It has been estimated that the cause of dysphagia can be determined with an accuracy of approximately 80% on the basis of a careful history alone.2 Some key elements of the history for patients with dysphagia are highlighted below.

Benign esophageal strictures

In most cases, benign strictures of the esophagus are presumed to develop as sequelae of deep esophageal ulcerations that stimulate fibrous tissue production and collagen deposition.38 However, the precise mechanisms that lead to stricture formation are not known, and it is conceivable that some strictures develop as a result of chronic esophageal inflammation even without mucosal ulceration.       A complaint of food “getting stuck” that localizes to the chest (ie, below the suprasternal notch) suggests esophageal origin. Localization to the neck is less helpful, as frequently patients perceive the symptoms at levels higher than the anatomic deficit.2,16,26 Concomitant heartburn, chest pain, and odynophagia also suggest an esophageal origin.15
